The Movie Starts with Milo Stanfield, a man diagnosed with autism spectrum disorder (ASD), who is observed by the audience as he stands on a street in New Jersey. He fixates on a woman in a nearby flower shop and also appears to be performing complex calculations related to the timing of events around him. Milo notes the approach of a passenger bus, a pothole, a street artist, and a cyclist speeding by. He strategically places an esa pen on the top of a mailbox near the pothole, and then sets into motion his planned chain reaction. A taxi runs over the pothole, splashing water onto the pen, causing it to fall to the ground. This catches the attention of the street artist, who then walks forward to pick up the pen. At the same time, the speeding cyclist collides with the artist, causing him to strike a nearby mailbox and fall onto a fruit stand. Chaos ensues on the street, diverting everyone’s attention, including the bus driver, who fails to notice the traffic light turning red and hits the same woman crossing the road, killing her on the spot. People say that math isn’t dangerous, but Milo walks away from the scene after confirming that his plan went smoothly.
The scene then shifts to FBI Special Agent Olivia Dunham, who returns to work after a long absence due to a head injury. Her colleague Charlie greets her upon her arrival at The Fringe Division of Homeland Security, which specializes in paranormal phenomena. Philip Broyles, one of Olivia’s fellow officers and lab assistant, expresses concerns about Olivia’s return as she is an impostor named Faux Olivia. Philip wonders if they can integrate an impostor into their team. Walternate Bishop, Chief of the Agency, reveals that Olivia is necessary because she is the only one who can travel between universes without any harm. Bishop aims to figure out how she can do so for this purpose. Charlie briefs his team on a seemingly random bus accident that has occurred twice in one week. The victim has died in both cases. Charlie asks his crew to be cautious before going for site investigation.
The FBI team arrives at the accident site and splits up to search for any leads or clues. Agent Lincoln Lee leads the investigation while Olivia questions the bus driver. Meanwhile, Agent Lee tests for any signs of environmental factors but finds nothing; however, he does find an identical ballpoint pen at the scene after questioning eyewitnesses. Olivia learns that the local artist had bent down to pick up the pen during which he collided with a bicycle rider and fell into a fruit stand upon hearing this; Agent Lee speculates that this pen is responsible for creating a sequence of reactions ultimately leading to the victim’s death.
The next day at French headquarters, Agent Lee and Charlie are trying to find out a connection between two bus accidents that have occurred within two days. Agent Lee spots an identical ballpoint pen in a photo of the first accident; this makes him even more convinced that someone has orchestrated this chain of events twice in succession. Following this discovery, they visit Astred, a mathematician at FR Agency, and inform her about their findings; however, Astred claims that no human can carry out such a chain of events and mold it in his or her favor twice amidst their argument another similar bus accident takes place leaving ASTD in shock two plus two equals you get hit by a bus. The FBI agents immediately rush to the site and find that both victims are still alive; this time upon questioning bystanders they learn that a dog suddenly crossed the road causing the bus to lose control just then Olivia notices an identical ballpoint pen rolling down from somewhere which makes her realize that this chain of events is not over yet; she looks around and spots Milo looking down from an overhead Bridge moments later; a man wearing earbuds walks forward to cross the street only to be hit by an oncoming ambulance I guess paramedic needed to hit his quota with her suspicions confirmed Olivia alerts her team and races towards Milo seeing this Milo jumps off the bridge lands on the bus’s roof and gets away Milo is known as “the Bus Whisperer” after this event Milo returns home to find his sister Mateline worried for him; she tells him that she has been getting numerous calls from hospitals but he ignores her; she then tries to convince him to surrender claiming that he is gone way too far however Milo anticipates her actions and completes her sentences effectively silencing her not long after Mateline shows him a small plastic horse toy-a reminder from their mother-this makes him feel uneasy and he walks away saying that it is irrelevant now onwards; Agent Lee is taken to hospital for his regular skin checkup before entering burn chamber he asks Olivia to focus on finding commonalities among three victims heeding his instructions
Olivia returns to headquarters and interviews the witnesses of the accidents. After some investigation, she uncovers connections between the three victims and a medical center. All three were directly or indirectly related to this center. She shares her discovery with Charlie, and they immediately head to the medical center.
Upon arrival, Olivia notices that the patients at the center are using the same ballpoint pens as they are unable to cope with digital devices. Later, they meet Dr. Levan, the chief doctor, who recognizes all three victims of the bus accidents. Dr. Levan explains that they have been conducting experimental processes to boost the intelligence of mentally challenged patients and shows them video footage of their patients, including the sighting of Milo.
Dr. Levan reveals that Milo was one of their patients who had gone through an experimental drug trial that significantly boosted his IQ. He mentions that Milo was released from the hospital under supervision but is now not responding to their calls and is committing crimes. Upon learning about Milo’s sister, the FBI agents decide to visit her. Unbeknownst to them, Milo is keeping a close eye on Olivia, noting her every movement and predicting her next move.
Charlie and Olivia arrive at Milo’s place and speak to his sister, Mateline. She claims to be unaware of Milo’s whereabouts, but upon learning about the crimes he has committed, she opens up about his abilities and how to break his concentration by showing him a toy horse. Mateline eventually discloses Milo’s current location, and Olivia and Charlie discuss plans to capture him with ASD.
They realize that any plan would be futile because of Milo’s predictive abilities and decide to approach the hotel directly without any backup. Meanwhile, Milo is seen at his hotel room predicting the arrival of Olivia and Charlie and planning to lure Olivia to a dangerous location.
At the construction site, Olivia races through the thin air zone and avoids the pile of cemented bricks that Milo had calculated to crush her, ultimately outsmarting him. However, she nearly asphyxiates herself due to the low oxygen level, but Charlie arrives just in time to assist her.
Later, Olivia brings Mateline to the hospital where Milo is interacting with a supercomputer. The doctors explain that Milo’s brain has been affected by the IQ-boosting drug for too long, and only machines can comprehend his thoughts now. Saddened by this news, Mateline leaves a toy horse in front of her brother.
That evening, Olivia experiences a vision of her boyfriend, Peter, from another universe, who encourages her to recall her memories and kisses her before vanishing.
